ISEF (International Science and Engineering Fair) Forms - for ALL students

Also known as SRC (Scientific Review Committee) Forms

We are affiliated with state and international fairs, and therefore use the forms compliant with the international rules for science research and science fair competition. These forms are used for students of all grades and levels of competition, from school and local fairs to regional, state, and international competitions.

Form 1 Instructions from our SRC chair - PDF.

An explanation of each form can be found on the Intel ISEF website.

Note: the signature date is the date of project approval, not the date of signature.

TO ALL SCIENCE FAIR TEACHERS, ADVISORS AND STUDENTS FROM THE REGIONAL SRC (Scientific Review Committee) (printable PDF)

For Project Registration, please save our volunteers time by organizing your forms in this order (top of pile to back) AND please print forms SINGLE SIDED (NOT duplex):

  1. Check made out to TCRSF for registration fee (& electricity fee if you requested it; this covers the cost of running cable) - one check may be used for multiple fees, but please indicate what students, projects, papers, or electricity fees are included in that check)
  2. Media release form signed by parent (one for each student) - This can be printed or emailed after registering online.
  3. Form 1 - Please make sure all appropriate boxes are checked and date is BEFORE your start date listed on form 1A. We need the date to show the date the adult supervisor approved the project (said it was okay for the student to do this project), NOT the current date the form is being filled out.
  4. Form 1A
  5. Complete Research Plan  - this is just a typed document that includes: A.) Statement of the problem or question being investigated, B.) Hypothesis, C.) Detailed procedure(s) - (We need to know concentrations of chemicals, sources of cell cultures or genetic material or sources of tissues used, and your analysis plan.), and D.) Bibliography or References: MINIMUM of 5 good references - and if you use a website as a reference include the date accessed because websites may change. If you have used one or more vertebrate animals, then at least one reference must include information on the care of that vertebrate animal.
  6. Form 1B - This must be dated before the start date listed on Form 1A. We need the date to show the date the parent and student knew the risks concerning their project and approved that they could do this project (parent said it was okay for the student to do this project), NOT the current date the form is being filled out.
  7. Forms 1C through form 7 IF NEEDED, and certification of training letters or letters from university IACUC documentation, etc.,  as indicated on the Form 1 checklist and as determined by the procedures the student used. Form 7 Continuation is for ALL STUDENTS WHO ENTERED A REGIONAL SCIENCE FAIR in the previous year! With form 7 you must include last year's complete research plan and last year's abstract - clearly labeled in the upper right corner with LASTNAME YEAROFPROJECT.  E.g. Smith, John - 2010, Smith, John - 2009, Smith, John - 2008. We need the research plan for last year's project and the abstracts for last year's and all previous year's projects done at the regional or above levels.
  8. Abstract - This may be on the official ISEF Abstract form OR it may be a document on plain paper, but please include the student's first and last name, the title of the project, and the grade the student is in (grades 6-12). MAKE SURE THE WORD COUNT IS NO MORE THAN 250 WORDS!!!  NOTE: We require the abstract to be in a minimum of three places: entered on-line for each event the student is registered (paper, project, or both), AND a printed copy must be included in the forms paperwork, AND the abstract must be displayed vertically at the student's exhibit. (For regional & state fairs, the abstract may be on the board (on the ISEF form is optional), or in a plastic document holder or frame on the table top of the exhibit display, or taped to the front edge of the table; for ISEF, the abstract may NOT be on the boards but will be displayed vertically.)

For Paper Registration, mail the media release form signed by parent (one for each student) - This can be printed or emailed after registering online. AND the registration fee check made out to TCRSF. (One check for both paper & project or for multiple students is just fine as long as you indicate what entries and students the check covers.)  Students must also email their research paper and abstract AND enter their abstract into the online registration form. See directions on the "Research Paper" page of our website.
